Case Managers
Case Managers play an important role in protecting and supporting you and your family as you navigate through life's changes and our complex healthcare system. Case Managers are professionals committed to ensuring that patients receive the best care possible while saving the patient/family and insurance plan money. Your Case Manager is there to facilitate necessary clinical requirements and network families with resources specific to individual needs. Case Managers also serve their clients by leading the team of physicians, caregivers, and families toward positive clinical and financial outcomes.
Case Managers go through a certification process that evaluates competencies in areas such as patient assessment, care coordination and planning, patient monitoring, and evaluation of care. Along with certification, Case Managers have a clinical degree or background that allows them to take complex cases and use all available resources to ensure great outcomes. Additionally, Case Managers must have a significant number of Continuing Education hours to remain certified. The certification process ensures that Case Managers have the necessary skills to assess support systems and needs, then intervene as needed. Case Managers are in place to determine where there is a gap in service, provide appropriate interventions, and work toward a solution that benefits the patient, family, care providers and insurance company.
Routinely, Case Managers are employed by your insurance company. Although, not all patients with a bleeding disorder are assigned a Case Manager. Due to the fact, the insurance company employs the Case Manager; the misperception is that cost is the only variable considered by the Case Manager. However, that is far from reality. Case Managers are in place to be advocates for patients and families. The standards by which Case Managers adhere to are based in ethical and appropriate care.
